The set up is a cinch. Just shuffle the deck and set it in the middle of the table. Each person will need to reach the deck without trouble. It is also important that everyone can see the deck since they will all have to see the cards being drawn to play.
The point of this game is to make a connection. A connection is made through matching cards with the person before you. The first player will draw a card and place it face up. The next player draws a card. If their card matches the number or the suit of the player's card before them then a connection is made. Players continue drawing cards to make connections.
When a connection is made players must drink according to the type of connection. If it is a number connection where the player's cards have matching numbers then both players must drink for that number of seconds. If the connection is a suit match then each player drinks for the number of seconds according to their card number.
The game ends when all the cards are drawn from the deck.
In some games connections can be made by multiple players. For example, if player one draws a queen of hearts, player two draws a seven of hearts then they make a connection and drink. Then if player three goes and draws a heart all three players must drink again because another connection has been made.
Another variation is to deal all the cards out to players at the beginning of the game into piles for each player. Players can't look at their cards. Each player then draws from their own pile and the game continues as usual. This is good if the group is large or there is no way for everyone to reach one deck. There is a variation on this, though, where everyone flips the top card of their deck at once and must drink for every connection made.
